September is upon us, and with it, our annual celebration of National PACE Month. PACE, which stands for Program of All-Inclusive Care for the Elderly, is a comprehensive, community-based model of care designed to provide aging adults with the care and support services that can help them remain independent and live with dignity in their homes instead of in a nursing home.

This year's theme, "Simplify Your Life: At Your PACE," perfectly aligns with the PACE program's mission. It emphasizes PACE's role in simplifying care by streamlining and delivering all necessary services.

More than 50 years ago, On Lok was founded in response to a community need. We created the PACE program and today, On Lok PACE serves over 2,100 seniors in San Francisco, Southern Alameda, and Santa Clara counties, while 77,000 frail seniors are served nationally by 171 PACE organizations across 33 states.

As our population ages, the need for comprehensive and compassionate care solutions becomes increasingly importance. PACE provides a continuum of care for older adults, who are eligible for nursing home care but wish to remain in their communities. On Lok PACE provides seniors in San Francisco, Santa Clara, and the Tri-City area of Alameda County with a full-service healthcare program that addresses the complex needs of seniors, offering everything from primary and specialty care to transportation and meal services.

The impact of PACE on seniors is profound. Participants often experience improved health outcomes, greater independence, and enhanced quality of life. Families also benefit, as they can have peace of mind knowing their loved ones are receiving comprehensive and compassionate care.

National PACE Month is a time to celebrate the remarkable achievements of the PACE program and to recognize the dedication of the professionals, caregivers, and communities that make it possible. By providing comprehensive, community-based care, PACE is making a real difference in the lives of older adults and their families.
