What to Expect During the On Lok PACE Enrollment Process

Step 1: Consultation with enrollment and outreach representative

Our enrollment and outreach representative will meet with you to learn more about your needs and answer your questions about On Lok PACE.

Question 2

Step 2: Eligibility review with intake team and submission for state approval

Answer

Our nurse will visit with you to learn about your health and social supports. We will also complete an evaluation to determine your eligibility for On Lok PACE and submit it to the state for approval.

Question 3

Step 3: Care plan development and enrollment conference

Answer

Once approved by the state, our team will create a personalized care plan to meet your care needs. We will also meet with you to discuss the care plan and review the enrollment agreement and other paperwork to help you enroll in On Lok PACE.

Question 4

Step 4: Participant enrolled

Answer

On the 1st day of the following month after you sign an enrollment agreement, you can start enjoying the benefits of On Lok PACE.
