PACE services include in home care

If you or your loved one needs additional care at home, On Lok PACE includes in home assistance with activities of daily living.

Our On Lok PACE home care caregivers are professionally trained to understand and assist you with your day-to-day care needs, including working with seniors who have memory support needs and dementia. They can help with:

• Light housekeeping

• Personal care

• Light meals preparation

• Feeding

• Medication reminders

• Laundry

• Escort assistance to medical appointments, as needed

• Preparation for van transport to and from PACE center visits or other health care appointments

Home care is a supportive senior service that may be included in your personalized care plan with On Lok PACE.
