On Lok PACE: Empowering you to live your best life

On Lok’s founders created the Program of All-Inclusive Care for the Elderly, nationally known as PACE, to provide seniors who wanted to stay in the community with the care and services they needed to age safely in place, instead of a nursing home. 

What we believed then, and advocate for still today, is that everyone should have the choice to age at home, regardless of their physical, medical, and financial circumstances. With their medical and long-term care needs taken care of, On Lok PACE participants can live to their fullest, at any age and level of ability.

The benefits of On Lok PACE for you or your loved one

On Lok PACE makes aging at home possible by:

  • Working with you and your family to design a personalized care plan.

  • Carefully assessing your needs and managing every aspect of your health care – using a high-touch approach that makes you feel valued and supported – by an 11-member interdisciplinary team.

  • Providing you support at home, as well as transportation from your home to our clinics and adult day health centers, and to our network of more than 1,600 providers.

  • Assisting with the tasks of daily living, such as preparing a meal or doing laundry.

When they learn about PACE, most people are surprised there is a senior program that takes care of everything. But for the past 50 years, we have been relentlessly pursuing quality of life and quality of care for seniors and their families, every day.

A team of experts in senior care that you can lean on

As an On Lok PACE participant, you can count on a healthcare team that includes: 

  • Primary care providers

  • Registered nurse

  • Social worker

  • Physical therapist

  • Occupational therapist

  • Activity therapist

  • Registered dietitian

  • Home care coordinator

  • Caregivers

  • Drivers

The professionals on your care team are selected for their experience and qualifications in senior care. They meet regularly to exchange information and review your plan as your needs change, handling all aspects of your care. They partner with you to schedule and coordinate appointments with our vast network of specialists, fill prescriptions, and provide transportation and support at home when needed. And, they are always available to answer your questions.
